Tyron was admitted to Sans Souci Rehabilitation and Nursing Center on August 20, 2016 from Montefiore Medical Center with a diagnosis of a pilonidal cyst with abscess, difficulty walking and diabetes. At the time of admission, Tyron was significantly deconditioned and as a result, he required extensive assistance to transfer while ambulating approximately five feet with a rolling walker. Tyron required extensive assistance with all functional mobility, activities of daily living and self-care due to underlying deficits in strength, dynamic balance and functional endurance. Additionally, Tyron demonstrated poor safety awareness due to impaired executive functioning resulting in poor judgment and impulsivity.

Tyron adapted to Sans Souci well and began making significant progress immediately. He improved in all clinical parameters that enabled him to have a safe discharge home. Tyron successfully rehabilitated to his prior level of function and even made a few advancements. Tyron was discharged home on October 21, 2016 ambulating unlimited household and community distances with a single point cane, ascending & descending 12 steps while displaying good balance and functional endurance. Tyron would like to thank the Entire Interdisciplinary team at Sans Souci as without them; His progress and where he is today would not be possible!
