Details of Experience:

The patient presented with an interesting and challenging request to be rehabilitated and safely discharged back home within ten days per their diagnoses. Noted as a relatively “younger” resident in age, their goal was to achieve their clinical goals over a holiday period in efforts of returning home as quickly as possible.

The interdisciplinary team at The Grove viewed this patient’s request as an excellent opportunity to provide optimal levels of care in hopes that the patient’s wishes could be fulfilled within the ten day time frame of inpatient care. The comprehensive care plan created on behalf of the patient was constructed to project a potential discharge date of within ten days and as in the case of all under the care of The Grove, the care plan was carried out to respect the patient-centered experience each patient is provided with when at The Grove.

The patient was supported to achieve more through the encouragement of fellow patients and staff. Within a mere 24 hours, the patient had connected with various departments within the community, including that of: the nursing staff, therapists, administration and social work.

The affect and atmosphere he was surrounded with was one of nurturing support and genuine care. He was all smiles for the duration of his therapy stay, and even ended requesting an extension of a few days to be able to spend some more time with his therapists. We wish this patient an incredible continued journey of renewed health!
