Who doesn’t love a keyboard player? We were excited to welcome Rene Gramonte, expert keyboardist, to The Emerald Peek Rehabilitation and Nursing Center on February 27, 2017 from NewYork Presbyterian. Mr. Gramonte was a patient at the hospital due to a broken left leg. Mr. Gramonte also had a history of bone marrow cancer.

While recovering at the hospital, Mr. Gramonte and his family had several options for short term rehabilitation. Being a long term Peekskill resident and after visiting with Ivy Banach, our Clinical Liaison, Mr. Gramonte chose The Emerald Peek Rehabilitation and Nursing Center for location to his family and the renovated rehabilitation gym.

Katie Cullum, our Patient Concierge, greeted Mr. Gramonte upon his arrival, welcoming Rene and his family to The Emerald Peek. Our clinical team, including our RN Care Manager, Unit Manager and Directors of Rehab, Dietary, Activities and Social Work met with Rene during his first day. Each team member welcomed Ms. McCabe and took the time to find out what each could do to make his experience exceptional at The Emerald Peek. It was during those conversations that we discovered Rene is a world class keyboardist, so a table top keyboard was made available to him within an hour of arriving. In addition, the team learned of Rene’s love of nature and drawing, particularly portraits. The team also made sure Rene had art supplies available to him so he could draw whenever he became inspired to do so.

Within 2 hours of admission, our Rehab team partnered with Rene and got to work. Tosin, our occupational therapist, determined that toileting, grooming and lower body dressing were the areas of focus, while in physical therapy, Khristine and Jose honed in on bed mobility, stand and pivot transfers and independent dynamic standing. Within the first seven days, Rene improved to a supervision only goal with all three areas of OT and PT. In addition to Rehab, Rene maintained full participation in Therapeutic Recreational groups and activities, meeting other musicians and artists within the community. Rehabilitation and activities determined it was time to move on to long term goals for Rene.
For PT and OT, Rene’s long term goals were lofty. Khristine set supervision goals of transfers, ambulation and 10 stairs within three weeks time. As for OT, Tosin knew Rene could complete all grooming tasks, including at the sink, complete body dressing and toileting independently without supervision. Reaching these long term goals in a short amount of time take state of the art equipment, dedication and commitment not only from the rehab team, but the patient. Rene maintained his focus and was able to complete all of these tasks without supervision, including ambulating over 50 feet and climbing nearly 15 stairs independently. A true testament to the partnership the therapy team at The Emerald Peek developed with Rene! While working so diligently in therapy, Rene was also drawing portraits of residents within the community.

Rene was discharged home on March 25, 2017. As he was leaving, Rene complimented many of our staff, including the therapy team. Rene shared, “His (Jose) constant attention to my therapy needs gradually egged me to do better every day until today (3/24/17) when (Jose) said, ‘Ok, now you are a graduate!’”. We wish Mr. Gramonte all the best and were honored to be part of his recovery journey!
