How did this patient hear about Sans Souci? Hospital social worker

How did this patient hear about Sans Souci Rehabilitation and Nursing Center? He had a family member who was a patient.

Dorothy was admitted to Sans Souci on April 27th, 2018, after suffering a stroke. The stroke affected her left frontal lobe, which affected her functionality and capabilities to perform necessary activities of daily living (ADL’s), such as dressing, using the restroom, and personal care. She required maximum assistance with regard to toileting, bathing and dressing her lower body, whereas for upper body dressing she required minimum assistance. Upon her admission evaluation, she was assessed and determined to need both occupational and physical therapy. The goals set were to strengthen her upper and lower body muscles, as well as to enhance functionality in her activities of daily living. She was to practice balance exercises, stamina and endurance training, and transfer training.

Throughout her time at Sans Souci, Dorothy worked with her therapists on a variety of exercises. To accomplish the goals set forth at the onset of her rehabilitation program, she exercised therapeutic exercises to help her get back to her prior level of function. She worked on her dynamic standing balance to learn to function at home while standing. She worked on her safety awareness to help prevent any future injuries, and exercises to enhance her endurance and stamina. In addition, her occupational therapist also worked with her to perform activities of daily living and her physical therapist worked on transfer training.

As the time came for Dorothy to be discharged, she was functioning at a much higher level than upon admission. She was able to bathe with minimum assistance, was able to dress with just supervision or contact-guard assistance and was able to use the restroom with only stand-by assistance. This enabled her to return home and resume her life in the same fashion as she enjoyed prior to her accident.

We here at Sans Souci are beyond excited to see her return home to her friends and family. We wish only the best of health and happiness for Dorothy in the future!
