How did patient hear about Quality Rehabilitation Center: Lebanon Health and Rehab Management

Details of Experience:
On April 1, 2019, around noon, Mrs. Lawrence was sitting in her recliner watching television. She had just finished having her breakfast when she felt the urge to use the restroom. She grabbed her walker as usual and headed to the entry of her bathroom. Unexpectedly, she started to feel very lightheaded. She stated that she continued to the restroom and made it in time.

After she finished using the restroom, she stood up to wash her hands. All she remembers is waking up with a knot on the back of her head. She pushed the button on her life alert to get help. Her left foot was stuck between her toilet and her vanity, she was able to get it to release.

In about 3 minutes Life Alert responded and asked if she needed help, she replied, “Yes, I have fallen and can’t get up.” They sent a medical team, which arrived in 5 minutes to assist her. They then took her to Tennova Hospital.

Tennova Hospital decided to keep Mrs. Lawrence overnight for observation. She received a CT scan, as well as X-Rays. During the medical examination, they found that she had an irregular heartbeat. Dr. Gray Spoked with her and told her that she needed a pacemaker. They later completed an operation and placed the pacemaker to support her arrhythmia. After a few days, she was still complaining about her hip, so they set her up with physical therapy for balance, control, and pain management.

She received physical therapy for the remainder of her two weeks stay at Tennova. She was then discharged to Lebanon Health and Rehab, on April 12, 2019. While at Lebanon Health and Rehab, she continued receiving physical therapy. She remained at Lebanon Center until May 8, 2019. Before being discharged, she was referred to Quality Center for Rehabilitation and Healing. On May 8, 2019, she was admitted to Quality Center. While at Quality Center, she received physical therapy to support walking exercises, standing balance and stamina. She received therapy 3 times weekly with her therapist, Steven. Steven continued to work with her throughout her stay. Before she was discharged, she was able to walk with her walker – a goal of hers! She was also able to bathe and dress and support increased gains in regard to her activities of daily living. She was discharged from Quality Center on October 19, 2019, to Elmcroft Assisted Living, where she now resides. We wish her the very best and continued health and happiness!
