Details of Experience:
Ms. Eleanor George arrived at Sans Souci Rehabilitation and Nursing Center on August 29, 2019, from St. John’s Riverside Hospital. Prior to coming to Sans Souci, Ms. George suffered a fall that led to a fracture on the right side of her pelvis. As a result, Ms. George was unable to walk and required a wheelchair for ambulation. Upon completion of her hospital stay, Ms. George was referred to Sans Souci for physical rehabilitation to regain independence walking, navigating stairs, and performing activities of daily living (ADL’S).

Upon admission, Ms. George was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Ms. George to our community and conducted evaluations to custom tailor her stay.

Ms. George was evaluated by members of the rehabilitation team, Dexter Galette COTA, and Sam Beloni DPT, who performed an initial evaluation for her. Ms. George’s rehabilitation goals were to safely perform bed mobility tasks and functional transfers independently, ambulate 250 feet with a rolling walker, and safely ascend and descend 8 steps on the stairs. Ms. George’s evaluation revealed that she required maximum assistance for the completion of her therapy goals.

Ms. George appreciated the care she received from her nurses, Jessa and Jacinta, and built a close relationship with them during her stay. Ms. George also enjoyed being visited by Omar Garcia from the recreation department and worked together with him on artwork during his visits.

Two weeks into her stay, Ms. George had made significant strides in her rehabilitation journey. Ms. George was able to complete her bed mobility tasks with minimal assistance, was performing ADL tasks such as dressing with moderate assistance, and could walk 25 feet with a rolling walker with moderate assistance.

Ms. George always had a pleasant demeanor and greeted everyone with a smile. Ms. George enjoyed the luxury amenities at Sans Souci like their signature Snack Cart Program provided by the community Concierge and daily newspaper delivery by the recreation team.

Ms. George continued to make significant strides throughout her stay. She was upgraded from utilizing 3 liters of portable oxygen to 2 liters of oxygen by a respiratory therapist, Terrence Seymour, one month into her stay. Ms. George could now ambulate 90 feet with a rolling walker with minimal assistance, ascend and descend 4 steps on the community stairs with moderate assistance, and could perform ADL’s and transfers with minimal assistance.

The week prior to discharge, Ms. George was able to perform bed mobility tasks and functional transfers independently. Ms. George was able to ambulate 350 feet, ascend and descend 15 steps, and perform ADL’s independently. At discharge, Ms. George shared “thank you to all the staff that assisted in caring for me. I came to Sans Souci on my back and now I am leaving walking again. I appreciate all that everyone at Sans Souci has done for me and for helping me get better!”
