Reason for Stay: Claire Dabeningo arrived at The Willows for short term rehabilitation due to a recent femoral fracture, secondary to a fall.

Details of Experience:
Claire Dabeningo arrived at The Willows Rehabilitation and Nursing Center from Westchester Medical Center for a bilateral femoral fracture. Upon entering our community, she was greeted by our interdisciplinary team, including our Director of Concierge Services, Yehuda. He welcomed her into our community and escorted her to her room. In addition to the fracture, Claire had a history of Osteoporosis, some muscle weakness and a left heel wound. She was non-weight bearing on her fractured leg, which made transferring very challenging. The therapy team evaluated her shortly after her arrival and formed a treatment plan and set goals she would need to achieve for her to return back home.

The journey was long for Claire and the first days in therapy were difficult. Until her non-weight bearing status was lifted, Physical Therapy worked on strengthening her legs with individualized modalities. Exercises and activities done with weights and resistance helped move her progress forward. When her non-weight bearing status was lifted, the focus became ambulation, standing and balance, with modalities focused on fall prevention.

The Rehabilitation team also focused on building upper tolerance and strength to help enhance her Activities of Daily Living (ADLs). Occupational Therapy worked on returning Claire to her prior levels of functionality. She wanted to be as independent with these activities as physically possible as her independence was important to her, including getting dressed, toileting and personal hygiene with little or no help.

Claire always shared a positive outlook with the team and her fellow patients. She had a smile and a compliment for everyone. She particularly enjoyed her dining room experiences, sharing meals with her friends and participating in the activities presented by our Recreation Team.

As her time at the Willows was coming to an end, Claire was able to perform her ADLs and walk longer distances with minimum supervision. The team worked with a home care agency to ensure a continuity of supervision at home. Once this support was put into place, Claire was prepared for discharge. Claire’s experience at The Willows enabled her to return home stronger than she expected. She was able to reach her maximum potential, allowing her to be comfortable and safe at home. The team at The Willows was honored to care for Claire and wish her much success in her life’s journey.
