How did this patient hear about The Paramount at Somers? Case Management at Phelps Hospital

Details of Experience:
Thomas was living a fully functional life, complete with commuting each day to his full-time job in Manhatten. His independent living was interrupted, however, when he developed sores on his feet, which progressively worsened over time and made it difficult for him to walk. After a series of hospital stays, Tom underwent an above the knee left leg amputation. He stayed in the hospital for a short period, before arriving at The Paramount, where he continued his road to recovery.

Though Tom had a long journey ahead of him, The Paramount was honored to be chosen as his place for rehabilitation. The community provides specialized rehabilitative amputee care through the highly acclaimed “State of the Art Comprehensive Amputee Rehabilitation Program”. The team is incredibly grateful for The Paramount’s partnership with Dennis and Todd, two phenomenal Paralympic gold medalists who have helped over 20,000 amputees worldwide. This program provided Tom with wonderful healing, mentoring, education, and training as he recovered.

Over the first weeks of therapy, Tom worked with the physical and occupational therapists for a personalized routine to regain strength and function. Guided by his therapists, he began receiving training on his prosthetics. During this time, Dennis and Tod visited him in person to provide mentorship and guidance to help Tom stay motivated.

Tom continued to gain his strength and made progress with bed mobility, balance, and transfers through January and February, by which he was able to

ambulate a distance of 20 feet with contact guard assistance by his therapist. On February 28th, his left leg was fitted for his prostheses. He continued his therapy with his new prostheses and was able to walk with a rolling walker and up and downstairs. On March 26th, he met all of his PT and OT goals and was able to safely discharge to home and to continue his therapy as an outpatient.

In the words of Kemi Sogunle, “The tests we face in life’s journey are not to reveal our weaknesses but to help us discover our inner strengths. We can only know how strong we are when we strive and thrive beyond the challenges we face”. Tom is a very strong individual who overcame a number of complicated medical conditions. The Paramount team considers themselves lucky to have been able to assist him on his journey, and wish him all the best moving forward.
