Details of Experience:
Charles is a 60-year-old gentleman who admitted to The Paramount at Somers for skilled physical therapy and occupational therapy. He came to us from Vassar Brothers Hospital with a primary diagnosis of acute respiratory failure with hypoxia and a secondary diagnosis of CHF. He has a history of CVA, resulting in left side weakness and a controlled left hand, along with a history of COPD.

Prior to his admission, Charles was previously living independently in his own private apartment. He was using a cane for ambulation and would enjoy venturing out in the community.

Upon initial evaluation, it was clear that he had a lot of rehabilitation to do before he would be able to care for himself. He presented with decreased self-efficacy, decreased strength, decreased activity tolerance, difficulty in performing functional mobility (i.e. bed mobility, transfers, and ambulation), was at risk for falls due to his unsteady balance and had decreased safety awareness.

Over the weeks, he showed slow but steady improvement. In the first few weeks, he began to show progress and continued with his occupational and physical therapy. As weeks went by Charles worked hard and progressed greatly. Just as he was almost ready to discharge, Charles had a few setbacks. He became ill and went to Putnam Valley Hospital for further treatment. When he returned to The Paramount, he was diagnosed with acute respiratory failure and an infection. As he resumed his rehab therapy, progress was slow going and he struggled with constant fatigue, he had no appetite and low motivation. This resulted in canceling his discharge and staying at The Paramount to continue his rehab. However, Charles continued to stay positive, did his best to overcome his obstacles, meet his goals, and get back that discharge that was taken away.

After a long struggle, Charles persevered and began regaining his strength. Rehab focused on improving his balance, endurance, and decreasing his risk for falls. Charles kept his positive attitude and was soon reaching his goals.

By mid-August, Charles made a huge change and made tremendous progress. His outgoing and funny personality had returned. He was now driven, motivated and determined. Charles met his short-term and long-term goals. He discharged independently with bed mobility, transfers and was ambulating with his single point cane. He was independent with his ADL’s, showed great endurance and strength where

he was able to ascend and descend 12 stairs with supervision. What motivated him the most was being able to attend his daughter’s wedding on 9/12/2020 and walk her down the aisle. On 9/11/2020, Charles was able to be discharged safely to his sister’s house and attend his daughter’s wedding the next day.
