How did this patient hear about Water’s Edge Rehabilitation and Nursing Center? Hospital Referral

Details of Experience:
Kenneth “Kenny” R. lives your average, everyday life at home with his wife, Virginia. After experiencing a bad fall at home, Kenneth found himself at Mather Hospital with a broken hip. After a short stay in the hospital, he was recommended to go to a sub acute rehabilitation center to regain his strength and work hard to be independent again. After careful consideration and recommendation, Kenny chose Water’s Edge Rehabilitation and Nursing Center at Port Jefferson.

Arriving on September 28th, Kenny was unsure what to expect. Upon arrival, he was escorted to his private room by our admission nurse. After settling into his new temporary home, he was greeted by members of our interdisciplinary staff. Kenny expressed a few of his inquiries regarding his stay on day one, making it possible for our team at Water’s Edge to ensure his stay was the best it could be.
Kenny was eager to get started with physical therapy because he was even more eager to get home. On his first day in our community, he was evaluated by his therapist and discussed with them his therapy goals. His goal was simple, “to walk and go home.” Kenny showed great potential for rehabilitation, because he was motivated to return to his prior level of function.

Upon arrival, Kenny needed moderate to maximum assistance on nearly every level, after being independent for his whole life. This included transferring from the sitting to standing position, moving in bed, dressing himself, and many other daily life functions. During his stay, he worked with therapy twice a day to ensure that he would get to where he wanted to be.

Kenneth worked with his physical therapist, Jamie, everyday doing plyometric activities for increased challenge to work on balance and strength. They even worked together to ensure he would be able to walk on unlevel surfaces like curbs, potholes and unlevel hills. Kenny also worked with his occupational therapist, Rebecca, on progressive resistance exercises, dynamic balance activities while standing, throwing/catching activities to facilitate coordination, and strengthening activities to increase functional task performance.

After spending four weeks in our community, setting a goal and crushing it; Kenny was ready to go home. His discharge day was quickly approaching and he expressed so much gratitude for our team at Water’s Edge.

We are wishing Kenneth and Virginia a very happy and healthy future!
