How did this patient hear about Water’s Edge Rehabilitation and Nursing Center? Hospital Referral

Details of Experience:
Ms. Surriya arrived at Water’s Edge Rehabilitation and Nursing Center on October 24, 2020, after spending some time in Stony Brook Hospital. Ms. Surriya had experienced a stroke in her home, where she resides in the basement apartment of her daughter’s house. Upon arrival at Water’s Edge, she was exhibiting a new-onset decrease of strength, reduced ability to safely ambulate, and increased need for assistance from others. Although Ms. Surriya’s primary language is not English, our team at Water’s Edge provided her with a language board to ensure she could communicate her needs. Our interdisciplinary team also made sure we were providing extra communication ability with her family, who was able to translate her requests if need be.

Ms. Surriya was greeted by Water’s Edge interdisciplinary team on day one of being admitted. She met with therapy, who analyzed her abilities and assisted in making a goal of where she wanted to be before returning home. Together, the team of therapists, the patient, and the family created goals including, but not limited to: bed mobility, safe transfers, ambulation progress, as well as performing activities of daily life functionally and safely.

Ms. Surriya worked daily with her physical therapist, Danielle, on therapeutic resistance exercises, balance training, gait training, and transfer training to increase functional task performance. Alongside her occupational therapist, Alyssa, she practiced exercises to facilitate independence in self-care tasks using exercises like bicep curls, chest press, shoulder flexion to facilitate strength for functional task performance. Together, they also practiced balance skills and throwing/catching activities to facilitate coordination.

Ms. Surriya was responding well to the treatment that the team at Water’s Edge was providing. She was progressing well and discharge was approaching. After putting in her all and working hard to get where she wanted to be, she was ready to go home. She was able to perform daily life tasks with her newfound skills, safely perform transfer’s and mobilize to and from where she needed to go. Ms. Surriya didn’t let the journey stop her from reaching her destination and she did just that!

Water’s Edge wishes Ms. Surriya and her family nothing less than a happy and healthy New Year!
