Details of Experience:
Mr. Martinez was admitted to Palmetto Subacute Care Center from Baptist Hospital on October 6th, 2020 after a few days of hospitalization, due to right-sided weakness. Mr. Martinez presents with acute infract L pons, along with thrombus seen in V4 segments of the vertebral arteries and in the basilar artery as per a brain MRI. He also presented with mild basilar occlusion and some aneurism. He was referred to Palmetto for decreased strength, decreased functional mobility requiring increased assistance during bed mobility, transfer and gait, poor endurance to activity, decreased static and dynamic balance.

Upon arrival, Mr. Martinez was greeted by the concierge and nursing teams to give him a warm welcome and ensure he felt comfortable. The rehabilitation team’s first evaluation was made shortly after his arrival and they created a specific treatment plan according to his needs and goals. The most important goal for Mr. Martinez was to be able to walk again using a rolling walker by himself. During his rehabilitation journey, he demonstrated the potential to be able to return to his prior level of function and strong family support that allowed him to achieve his goals and return to his former life. When he was discharged, he was able to walk 75 feet using a rolling walker on a level surface with contact guard assist and minimal assistance in most of his activities of daily living (ADL).

On January 14th, Mr. Martinez was discharged to his assisted living facility. He was very happy and grateful for the dedication and commitment of the team at Palmetto, who wish him all the best!
