Details of Experience:
Constance was admitted to The Enclave at Rye Rehabilitation and Nursing Center on March 2, 2021, from the Hospital.  The Enclave at Rye was recommended to Constance by her doctor at the hospital for rehabilitation.

Upon arrival, the interdisciplinary team greeted and evaluated Constance, including nursing, rehabilitation, dietary, social services, concierge, and recreation departments. Within 24 hours of Constance’s arrival, a personalized treatment plan was created by the team. Constance would benefit from physical and occupational therapy. 

Lauren, her physical therapist, and Tim, her occupational therapist, met Constance and performed their initial evaluations and preliminary goals. She presented with balance deficits, gross motor coordination, and muscle weakness, severely impacting her ability to independently perform basic activities of daily living (ADL’s). She required physical assistance to ambulate 20 feet and needed extensive assistance with dressing, bathing and toileting. Goals were established to help her regain strength, while nursing carefully monitored the pain levels so that she could maximize her time in therapy.

Some of the goals included performing bed mobility tasks with Independence, ambulating 300 feet with a walker, and ascend/descend 15 stairs with modified independence.  The nursing department kept a close eye on Constance’s pain management, and managed her high blood pressure, and provided medical management for DVT, enabling her to maximize her time in therapy. Constance enjoyed the recreational activities such as music, puzzles, games, and socializing with the other residents in the day room. 

After six weeks here, Constance’s progress was remarkable. She met and surpassed all of her goals, took care of all ADLs, and was ready to adjust back to life at home. On April 14, Constance walked out of the Enclave with renewed strength and independence.

Her confidence in her health and independence was at a new high, and she was super excited to go back to her community. All the residents now miss her smile and upbeat presence!
