Details of Experience:
Patricia Adams was admitted to The Grand Pavilion for Rehabilitation and Nursing in March, 2021 due to diverticulitis and disorientation. Mrs. Adams presented to The Grand Pavilion with poor functional strength, impaired balance, poor functional endurance, and the need for rehabilitation therapy. Upon her admission to The Grand Pavilion, Mrs. Adams was greeted by the community’s staff, assessed, and made comfortable. Therapy spent a great deal of time discussing her treatment goals and plans to get her back on her feet and a return to her normal activities of daily living (ADL), as well as returning to her family.

Upon Mrs. Adams’ admission to The Grand Pavilion, she was at risk for falls. The patient had a decrease in leisure task participation, as well as, a decrease in her independent participation with functional activities of ADL tasks. Therapy worked hard to assess her needs and developed a daily plan of therapy that enhanced, encouraged, and promoted skilled physical and occupational therapy services. The Therapists evaluated and supported her recovery of safety awareness, improved dynamic balance, increased functional activity tolerance, and increased body strength. They helped her to minimize her fall risks and also decreased her complaints of pain, while enhancing her quality of life. Motivated and determined, Mrs. Adams worked fiercely and effortlessly with the therapy team in reaching her target goals of recovery and success.

During her stay at The Grand Pavilion, she also provided great compliments regarding the welcome she received and the care, support, and encouragement the Grand Pavilion staff provided. She stated, “The staff was very supportive and helpful; Michael from Therapy was AMAZING, and The Grand Pavilion is an immaculate community.”

Mrs. Adams had a successful discharge from The Grand Pavilion in May 2021 and ranked the community a 10/10. The Grand Pavilion acknowledges her hard work and success through her phenomenal therapy experience and wishes her the best in her future health and healing.
