How did the patient hear about Bethany Center for Rehabilitation and Healing? Our marketer reached out to the case manager at the hospital. The patient had Bethany Center down as their first choice for a rehabilitation center.

Details of Experience:
Ms. Rose was admitted to Bethany after an eight-night stay at Southern Hills Medical Center. She was admitted to Southern Hills for recurrent falls and generalized muscle weakness. During her stay at Bethany Center, she participated in physical therapy, speech therapy, and occupational therapy to provide her the techniques and knowledge she needs to discharge home. 

Upon admission, she was greeted by the Care Transition Coordinator, her nurse for the evening, her CNA for the evening, and the Concierge. The Care Transition Coordinator and Concierge worked together to get Ms. Rose settled into her room, complete her initial evaluation, and present the welcome bag. Ms. Rose stated, “I already love this place, and everyone seems so nice here.” The next day, therapy went to meet Ms. Rose to complete their initial evaluation. Together they created some short-term and long-term goals for her to reach during her therapy journey. Some of those goals include: to safely perform functional transfers with minimal assistance, increase the ability to propel in a wheelchair safely – 75 feet, and tolerate standing for greater than 1 minute. 

Ms. Rose’s baseline was taken on 6/29/21, and the first target date to meet the goals was 7/12/21. She participated in therapy 5 times a week for up to one hour a day. Therapy immediately started working on transfers from bed to wheelchair and back to build up her strength. By 7/5/21, Rose increased from maximal assistance with cues in transferring to minimal assistance and cues. She was doing very well with transfers to and from the wheelchair. By 7/5/21, Rose was able to safely propel herself in the wheelchair up to 10 feet with modified assistance. She was doing very well with propelling herself but needed more assistance with turning and maneuvering around objects.  

By 7/12, Rose met all of her goals and continued working hard during her therapy sessions. She was scheduled to participate in therapy until 7/25 before she was discharged home on 7/28. Ms. Rose was determined to return home stronger than ever. Each day, she pushed harder and harder and never gave up. As her discharge date drew near, she could fully meet all of her goals, transfer safely, propel herself well over 150 feet and maneuver around objects better, along with many other tasks. Upon her discharge evaluation completed by the concierge, she stated, “I am so thankful for the entire Bethany staff and everything they did for me while I was staying here.”
