Peggy is an 89-year-old female with PMH significant for acquired Hypothyroidism, hypertension, COPD, and Anemia. She was admitted to Overlook Hospital after she experienced a fall, resulting in a pubic fracture as well as a fracture of the left clavicle.

Peggy was admitted to Chatham Hills Subacute Care Center on 03/08/2017 from Overlook Medical Center. The hospital’s physical therapy team recommended subacute rehabilitation for Peggy, where the social work team consulted Peggy to be discharged for rehabilitation at Chatham Hills Subacute Care Center. As such, Peggy cleared for discharged to SAR, pain controlled on Oxycodone IR prn and Tylenol prn.

Impressions – Physical Therapy Clinical Impressions: Upon arrival at Chatham Hills Subacute, it was noted that Peggy demonstrated a significant decline in overall functioning following her fall with a left clavicle and inferior/superior pubic rami fractures. Peggy required assistance for all mobility, transfers, ambulation, and for safety.

Skilled Justification – Reason for Skilled Services: Peggy required skilled physical therapy services to evaluate need for assistive device, facilitate (I) with all functional mobility and promote safety awareness in order to enhance patient’s quality of life by improving Peggy’s ability to perform steady gait.

Risk Factors: Due to the documented physical impairments and associated functional deficits, the patient was at risk for: falls, immobility, and decrease in level of mobility.
