Reason for Stay: Failed Revascularization

We had a resident that was admitted October,8, 2014, after being hospitalized with left lower extremity ischemia with a failed revascularization. The failed revascularization resulted in left above knee amputation. The resident was independent with all her activities of daily living and ambulated independently with a straight cane. Upon admission to Cortlandt Healthcare this resident was non-ambulatory and a maximum assist with transfers with all of her activities of daily living. After three months of intense nursing care, physical and occupational therapy and a new left lower extremity prosthesis, this resident was able to ambulate with a rolling walker, negotiate nine steps with two handrails with supervision and perform all activities of daily living with modified independence!!!
