How did this patient hear about the Emerald Peek? Family member lived locally.

The resident came to Emerald Peek on 7/21/16. She came to us as a young, below the knee amputee from the hospital. This resident is a brittle diabetic with end stage renal disease, unable to self ambulate or perform any ADLss independently. Several years ago, the resident’s brother passed away while in his late 20’s from complications associated with diabetes. Emerald Peek staff took an “all hands on deck” approach to her care. Dieticians, nurses, recreation, therapists, psychiatric services and even the maintenance men and housekeepers were focused on getting the resident walking and getting her home. Upon discharge, the resident was able to walk under her own power to a vehicle waiting for her.

Here are the resident’s baseline measurements and goals achieved to date:

Bed Mobility
Baseline: Moderate Assist of 2
Current Achievement: Self Transfer

Transfers
Baseline: Maximum Assist of 2
Current Achievement: Self Transfer

Ambulation
Baseline: Poor with assistance
Current Achievement: 200 feet with rolling walker and minimal assistance, including 4 stairs
