Details of Experience:

Patient previously admitted to Heritage Center in 2015 in which she and her family were extremely grateful for the care provided by the interdisciplinary team. She was so pleased with the care provided by the team here at Heritage Center that she decided to readmit in result of a history of falling, muscle weakness and difficulty walking.

Patient was discharged after a three month stay and was later readmitted to Heritage Center from another fall at home soon after she had left. Although it was discouraging for her to have to come back so soon, she knows she will overcome this obstacle with more physical therapy and the excellent care she will receive from Heritage Center for Rehabilitation and Specialty Care.
