Patient was admitted on 01/105/15 from St. Elizabeth Boardman Medical Center. Patient was at Oasis Center for Skilled Rehabilitation and Skilled Nursing care following a hip replacement.

Patient had a complicated medical history to include Colon Cancer (in remission), alcohol abuse with subsequent Liver Cirrhosis and a previous admission at Oasis Center from 10/29/12 to 12/08/12 for end stage liver disease and many other health issues.

Patient lives at home with support from Home Health Services. Patient maintained a positive attitude throughout his stay. He was very sociable, pleasant and well- motivated. Patient has strong family support and plans on relocating to Florida.

Patient was discharged to home on 2/14/15. At admission, he was unable to walk and throughout his stay he graduated to wheeled walker, to a quad cane, to ambulating independently without an assistive device. Excellent prognosis and recommendation made for Outpatient Therapy.
