Patient is a 93 year old female who was referred for PT/OT services. Patient had a humerus fracture from a fall at home. Upon admission, Evelyn was a MAX assist with all ADL’s, ambulation and had over all weakness and decreased balance. PT/OT set appropriate goals to prepare her to return home independently.

Eveyln was a star in therapy! She came without question daily and did above and beyond what was asked and did it with a SMILE! With time and dedication she quickly began to improve functionally. Her favorite thing was the Nustep, which she called HER bike! She would ride it diligently!

If it weren’t for all of those girls I wouldn’t be as successful as I am today. The nurses took good care of me. I loved all of the activities; they made my days fly by. I’ll miss all of you! At time of discharge, Evelyn was walking all over the building with a front wheeled walker with very minimal help, her balance was good and she was participating in all of her ADL’s with little help. Therapy took Evelyn and her son on a home assessment to determine her safety to return back home. It was decided that Evelyn had met her goals and would return home. We will all miss her greatly!
