Resident is a 70 yr old female which came to Platinum Ridge from AGH after having an aortic valve replacement. Residents post operative course was complicated by respiratory failure requiring mechanical ventilation. Resident had decreased strength, balance, endurance and self care abilities. She progressed from max assistance with all self care tasks to modified independence. Upon admission to Platinum Ridge she was able to ambulate to 15ft then progressed to 200ft with a walker, walking up and down 4 steps and perform all transfers with modified independence. Resident was successfully discharged to home with family.
