Eileen had stayed with us at Platinum Ridge before for a couple weeks for rehab. She had returned after falling at home and being in the hospital. When evaluated she required minimal assistance for all ADL’S and contact guard assist for transfers. Throughout her time here she progressed to set-up and modified independence with all ADL’S. She demonstrated increased safety awareness with functional mobility and transfers as well, while performing with modified independence. Eileen was issued adaptive equipment to assist with ADL’S. She was able to complete homemaking tasks with modified independence too! She demonstrated an increase with endurance and standing tolerance to better facilitate with all self care, safe transfers, and functional mobility tasks. Eileen was able to be sent home with modified independence and assist from her daughter. Thanks to the great team here at Platinum Ridge!
