How did this patient hear about Platinum Ridge? From family and friends (local in community)

Patient came in to Platinum Ridge on 11/28/2014 from Allegheny Valley Hospital after suffering from a fall, resulting in a traumatic lower leg fracture that needed after care assistance. Resident stated during her admission survey that she was nervous and unsure how things will go, that the staff seemed very nice here and that she hopes that he can be up and walking soon.

Resident assessment on 11/29 showed that she was scoring at a 57 on MBI charts. With this resident would manage but in a supportive environment. Needing assistance with activities of daily living by a number of community resources.

Resident was showing wonderful improvement but had to go back out to the hospital due to her legs unfortunately filling up with fluids. Resident was out for about a week and unable to continue with therapy during her stay.

When the resident came back from the hospital she stated that she was happy to be back and ready to get back in action on her road to recovery.

By the time resident hit here half way assessment evaluation his MBI at a 71 meaning he was able to join community or home still needing assistance from community resources.

By the end of her stay here at Platinum resident was deemed completely perfect to go home without any supervision in fact she was able to live independently in home or community.

Platinum Ridge would like to thank our Physical therapy department and nursing department for resident’s wonderful recovery.

I followed up with resident and she stated that she is doing wonderful .Despite the weather she looks forward to going out for exercise. She mentioned how she loves to be independent, but that she misses having our kind and caring staff to keep her company. She greatly appreciated all the care and help she received while here at Platinum Ridge. Her goal is to not slip on ice while walking and when it gets warmer she would love to visit us here and say hello.
