This patient was notified of Squirrel Hill Center for Rehabilitation and Healing by her boyfriend after researching skilled nursing and rehabilitation centers in the Pittsburgh area.

This patient arrived at Squirrel Hill Center for Rehabilitation and Healing on January 23, 2015.
Prior to admission at Squirrel Hill Center for Rehabilitation and Healing this patient had been hospitalized after suffering a severe vehicular accident, which caused extensive damage to her pelvis and resulting in need for a pelvic fixator to be utilized in the healing process. This also left the patient at a non-weight bearing state.

Upon admission to the facility a multi-disciplinary team of staff met one on one with the patient to establish an individualized Plan of Care, establishing therapy goals to be reached during care, answering any and all questions the patient had, as well as noting the patient’s personal rehabilitation goals and expectations regarding care for the duration of her stay as well.

Although this patient was unable to bear weight or participate in Physical therapy due to placement of the pelvic fixator equipment, the skilled team of Occupational therapy team at Squirrel Hill Center for Rehabilitation and Healing followed her closely. This knowledgeable team worked on assisting the patient with regaining the ability to do tasks such as bed to wheelchair transfers and wheelchair ambulation as well as other body strengthening exercises (At time of her discharge the patient praised facility occupational therapists for the work they accomplished and kindness they showed during her stay).

This patient was also followed closely by the facility’s Director of Concierge Services and assistant Director of Nursing; both answering questions and assuring that all requests of the patient were handled as soon as possible. Both of these staff members were also sure to follow-up to ensure optimal patient satisfaction and experience. The Squirrel Hill Center’s social services staff were also sure to order all medical items the patient would need upon her return home.

At 29 Days this patient was able to have her fixator removed and was able to return home to her boyfriend and two children and where she would require minimal care, as she was still in a non-weight bearing state (future physical therapy will be underwent after patient recovers from fixator removal surgery) . This patient was now able to safely ambulate in a wheelchair and no longer required a pelvic fixator.

Upon discharge the patient stated that she would recommend Squirrel Hill Center for Rehabilitation and Healing to others and that she will return to the facility when in need of future care and physical therapy services. The patient also noted how facility staff went out of their way to ensure she was comfortable and happy during her journey of healing.
