Ms. Eleanor Bentman was admitted to The Grand Pavilion on March 10th, 2018 from St. Francis Hospital. Ms. Bentman was experiencing a diagnosis of st elevation, myocardial infarction, acute chronic systolic heart failure and generalized muscle weakness and was in great need of rehabilitation. Upon arrival to The Grand Pavilion, Eleanor, met first with our Concierge and then continued to meet with Nursing, Rehab, Social Work and Dietary. Eleanor is a very outspoken and caring person. She expressed upon admission that she experienced a very scary procedure where they replaced a valve in her heart and that she was staying optimistic with the goal to return home safely

Eleanor’s highlight of the day is going to therapy and enjoying the interactions she shares with her
fellow patients and staff members. Eleanor would love to attend activities on both the first floor and second floor of the community. She enjoyed the sing along and bingo. When Eleanor was asked by our Concierge what her greatest recreational highlight was, she responded immediately “The Irish Children dancers that put on a great show for St. Patrick’s Day”.

Ms. Bentman was experiencing a decrease in strength, poor balance, difficulty performing bed mobility and transfers. She was experiencing a lack of strength preventing her from performing standing activities as well as reducing her functional activity tolerance. The goal of Occupational Therapy was to increase functional activity tolerance, increase safety awareness, facilitate sitting tolerance and posture control as well as working on developing compensatory strategies. It was the goal of Physical Therapy to enhance functional abilities, facilitate increased independence with ambulation/stair negotiation, to increase lower extremities range of motion, thus minimizing falls. Eleanor was extremely motivated and pleasant with the whole rehabilitation team and was highly motivated to achieve her goals.

Eleanor made slow and steady progress during time spent in skilled rehab sessions. Eleanor participated in therapeutic graded exercises, bicep and tricep curls, exercises with gymnastic ball, proprioceptive techniques while at the same time participating in strengthening activities and dynamic functional activities. Eleanor is now reaching the end of her stay at The Grand Pavilion and she is participating in a floor ambulation of 150ft on a daily basis and all of her goals are being met.
When our Director of Concierge sat with Eleanor to follow up on her experience here at The Grand Pavilion, she was extremely satisfied with the food and care she received from the Nursing team and was very appreciative to Social Work for keeping her in the loop in regards to her progress. She is most appreciative to both the Physical and Occupational Therapy, as they are allowing her to physically get on her and function the way she was functioning before her surgery.

With the work of the whole Grand Pavilion team, Eleanor made excellent strides in her recovery to her prior level of functioning and is reaching the point where she will be able to return to her prior living environment. Ms. Bentman is extremely appreciative and emotional for all the care she received from the GP team. Eleanor will forever be part of The Grand Pavilion family!
