How did this patient hear about Sans Souci? SJ Social worker
Details of Experience:
Oscar Was admitted to Sans Souci from St. Johns Riverside Hospital with a left knee Amputation on October 13, 2014. This resident also had a Diabetic foot after a left popliteal artery Cath, Lt. SFA angioplasty and stenting. / HTN, DM, PVD, MRSA precautions left foot.
Upon Admission to our Therapy Department, Oscar exhibited a new onset of decrease in strength, decrease in functional mobility, decrease in transfers, reduced ability to safely ambulate, reduced functional activity tolerance and increased need for assistance from others indicating the need for PT/OT to increase ROM and strength, increase functional activity tolerance, increase independence with gait and promote safety awareness. After much hard work from our team, Oscar was fitted for a prosthetic limb- Left leg. It took some time for Oscar to get used to his new leg, but after the effort that Oscar put in with the help of our esteemed Rehab team, Oscar was well on his way to a full recovery. When Oscar was discharged on October 21, 2015 he was able to safely ambulate on level surfaces 40 feet using Prosthetic limb and RW with Min (A) with normalized gait pattern, with continuous steps and with safety during turning 80% of the time with no falls and without LOB to increase independence with all functional ambulation. It was a pleasure seeing the smiles mixed with tears from Oscar and all the staff upon his Discharge from Sans Souci.
