Details of Experience:

Connie is an 87 year old female who presented to us with shortness of breath, heart failure, muscle weakness and difficulty walking. She was admitted to Chatham Hills Subacute Care Center after her stay at Morristown Memorial. Prior to her admission in the hospital, the patient was completely independent. Her atrial fibrillation was fully treated at the hospital and then was admitted here for rehabilitation in need of the following short term and long term goals:

  • Patient to safely perform functional transfers with Supervision
  • Patient will increase dynamic standing balance
  • Patient will safely ambulate of level surfaces
  • Patient to safely ascend and descend 10 stairs

After completing both physical and occupational therapy to attain all of the above mentioned goals, the patient made consistent progress. Her functional abilities progressed as a result of skilled interventions and the patient reached maximum potential with skilled services. Upon initial evaluation, she required maximum assistance for transfers and had increased difficulty walking requiring increased assistance. Patient was discharged having attained all of her goals and safely walking with a platform walker.

Connie was a pleasure to work with. Her positive spirit was a huge factor in her quick recovery. She accomplished all of her goals and was able to go back to independent living with visiting nurse services, 4 hours per day.
