Reason for Stay: Failure to thrive; Weakness
Cesira was admitted to Sans Souci Rehabilitation and Nursing Center on 4/25/16 with a diagnosis of hyperlipidemia, renal failure, Failure to thrive and a history of Dementia and Depression. Cesira also had a PEG tube inserted to help her with her eating and drinking. When Cesira was assessed by our therapists, she required max assistance for all her ADL’s. Our Dietitian met with the resident and she continued to refuse to primarily eat through her mouth. Through much work from our Rehab, Nursing and Dietary team, Cesira started to make slow progress. Approximately 2 months into her stay at Sans Souci, Cesira was able to get her PEG tube removed as she started consuming most of her meals making her PEG tube unnecessary. Our SLP worked with her as well to make sure she had an easy time swallowing and eating to ensure that she would never need the PEG tube again. On July 19, 2016 Cesira was discharged to her loving family as she only needed Minimal assistance with her ADL’s and she was able to ambulate 100 feet with CGA using a rolling walker.
