Mrs. B was admitted to Cortlandt Healthcare on November 23, 2016 in result of an inpatient hospitalization due to a fall, which resulted in a fracture of her left hip. Mrs. B also presented with a history of Atrial fibrillation, a previous transient ischemic attack and diabetes.
Upon evaluation by the physical and occupational therapy teams, it was noted that Mrs. B required assistance of one person with her activities of daily living (i.e. getting in and out of bed, dressing, bathing, toileting) as well as significant decrease in overall strength and balance. Mrs. B was placed on program to achieve her prior level of functioning prior to her hospitalization. Her goal was to develop independence in her activities of daily living and activity within her community.
After just four, short weeks of skilled physical and occupational therapy and skilled nursing services, Mrs. B achieved her therapeutic goals and was medically approved to return home at an independent level with the use of a rolling walker. We wish her nothing but the best as she continues to set her goals high and achieve them one step at a time.
