Pasqua is a 76-year-old female who admitted to Sans Souci Rehabilitation and Nursing Center on 5/12/17 from St. John’s Riverside Hospital for short-term rehab, as a result of a pubic rami fracture. This was in a large part caused by metastases of cancer to the bone.

At the onset of her rehabilitation program, she had minimal strength, decreased dynamic balance and functional activity tolerance; in addition to excessive pain which interfered with her bed mobility, functional transfers and ambulation. She required total assistance with her ADLs and IADLs. Over the course of her time here at Sans Souci, Pasqua proved to be a strong fighter. As illustrated by her previous battles with cancer, she would not let her illness and setbacks get in her way. Week after week she made continued progress in her therapy. She and her family met periodically with the Sans Souci team to tailor a plan that would enable her to succeed and win like she was determined to do. Despite undergoing chemotherapy, and being subject to the side effects that it brings, she adhered to all advice of the nursing department and the Registered Dietician, and she continued to thrive nutritionally.

As she approached her date of discharge, Pasqua had made significant progress. She was able to perform bed-mobility tasks, with only set-up assistance. She was able to transfer and ambulate more than 250 feet with a rolling walker, and only supervisory assistance with a wheelchair follow. Additionally, she was able to ascend and descend twelve steps with stand by assist, which she required in order to enter and exit her home. On 8/30/17, she was discharged home after a three and a half month stay. These monumental gains enabled her to return home to her loving family who missed her so much, and encouraged her every step of the way. As Pasqua told the Director of Concierge Services after her discharge, “This place is like family to me. Every staff member- from the nurses who cared for me every day, to the Administrator- were so warm and friendly, and made sure my every need was met. I can’t wait to walk back into Sans Souci as a VISITOR!” We are glad to see her return home and continue enjoying her beautiful life!
