Details of Experience:
Mr. Reyes arrived at Sans Souci Rehabilitation and Nursing Center on August 12, 2019, from St. Joseph’s Medical Center. Prior to arriving at Sans Souci, Mr. Reyes complained of muscle weakness and difficulty walking. Mr. Reyes underwent a total knee replacement to correct his knee joint. Upon completion of his hospital stay, Mr. Reyes was referred to Sans Souci for physical rehabilitation in hopes to begin walking again.

Upon admission, Mr. Reyes was greeted by multiple members of the Sans Souci interdisciplinary team. The team welcomed Mr. Reyes to our community and completed evaluations to custom tailor Mr. Reyes’s stay.

Mr. Reyes was evaluated by members of the rehabilitation team, Dexter Galette, certified occupational therapy assistant, and Samuel Ignacio, doctor of physical therapy, who performed initial assessments. Mr. Reyes’s initial rehabilitation goals were to be able to safely perform transfers independently, ambulate 250 feet with a rolling walker and improved range of motion, independently perform ADL’s, and ascend and descend 15 steps on stairs.

Mr. Reyes’s evaluation revealed that he was able to ambulate 25 feet using a rolling walker with supervision and required minimal assistance with functional transfers. Mr. Reyes was unable to ascend or descend stairs and required minimal assistance in completing ADL tasks.

During his stay, Mr. Reyes became close friends with his roommate who was also fluent in Spanish. Mr. Reyes enjoyed attending recreation activities, such as bingo, BBQs, and musician visits led by Recreation Director, Leduc Le and Recreation Therapist, Omar Garcia.

A week and a half into his stay, Mr. Reyes met his occupational therapy goals. He was now able to complete activity of daily living (ADL) tasks independently and could perform functional transfers with modified independence. Mr. Reyes made significant progress with his physical therapy goals as well! He was now able to ambulate 225 feet utilizing a rolling walker with supervision, and ascended and descended four steps on the community stairs.

Mr. Reyes shared with the community’s concierge that he was a veteran of the U.S. Army. Mr. Reyes enjoyed attending the monthly Veteran’s Coffee Social with other residents as well as staff veterans at Sans Souci led by our concierge, Aryeh Schachter.

Three weeks into his stay, the facility began planning Mr. Reyes’s discharge home. Mr. Reyes exceeded his ambulation goal and was now able to ambulate unlimited distances with a rolling walker independently. Mr. Reyes met his stair goal and was able to ascend and descend 15 steps on the community stairs with handrails.

Upon the time of discharge, Mr. Reyes shared “My stay at San Souci was a great experience. I went in with pain and came out pain-free. The staff took great care of me. The rehab team was great! I would recommend this place to anyone!”
