Details of Experience:
On July 7, 2026, Ava was admitted to Luxor at Sayville Rehabilitation and Nursing Center following a stroke for sub-acute rehabilitation. Her condition required specialized rehabilitation, attentive nursing care, and a compassionate, structured environment to support her recovery and foster independence. Ava was referred to our center by her hospital caseworker, who recognized that we could provide the comprehensive care she needed for a successful rehabilitation.

Upon arrival, Ava was warmly welcomed by our interdisciplinary care team, including Nursing, Physical Therapy (PT), Occupational Therapy (OT), Social Work, Recreation, and Concierge Services. Together, we developed a personalized care plan tailored to her specific needs and goals. Our primary focus was to help Ava improve her mobility, increase independence with activities of daily living, and build confidence in managing her daily routine. We wanted to ensure her every need was met and that her transition into our community was smooth and supportive.

Outside of her therapy sessions, Ava quickly became a cherished member of the community. She enjoys playing cards and has formed close friendships with fellow residents. Ava is highly involved in recreational activities and especially enjoys getting her nails done and playing bingo. These experiences created a strong social circle that became a key source of joy and motivation throughout her stay. Her positive attitude, combined with the unwavering support of her care team, supported her steady progress.

Throughout her sub-acute rehabilitation stay, Ava made meaningful strides in her recovery. She progressed from requiring maximum assistance with activities of daily living to becoming a one-person assist. She is now able to ambulate with the support of a walker, communicate more effectively, and tolerate oral nutrition. Her mood has improved tremendously, and she has become more engaged in both her care and daily activities.

We are incredibly proud of Ava’s determination and the progress she has achieved. It has been a privilege to be a part of her journey, and we wish her continued health, happiness, and success.
