Mr. Robert S is a 57-year-old male admitted to Trevecca Center on September 18, 2025. Upon admission, he expressed feeling overwhelmed; however, our team ensured that he understood he was in a supportive environment dedicated to his recovery and well-being.

On Day One, Mr. S was warmly welcomed by Director of Concierge, along with our admissions nurse. We assisted in ensuring he had everything needed for a smooth transition into the facility. Shortly after, our Rehabilitation Director and therapy team met with him, followed by an introduction to the Unit Manager, Marissa, to establish continuity of care.

On Day Two, the Director of Food Services met with Mr. S to discuss dietary preferences and menu options, ensuring his nutritional needs and preferences were addressed early in his stay. Our interdisciplinary team quickly established a comprehensive care plan. Mr. S required Physical Therapy, Occupational Therapy, and Speech Therapy. Our goals included helping him reach his maximum functional capacity, evaluating and treating cognitive function, promoting out-of-bed (OOB) tolerance, strengthening upper body mobility for functional independence, and encouraging participation in activities of daily living (ADLs). A long-term goal included preparing him for a prosthetic leg once he regained sufficient strength and stability.

In the early stages of his stay, Mr. S required full assistance with activities such as dressing, bathing, and transferring in and out of his chair. Despite these challenges, he remained socially engaged and participated in activities with peers in the dining area, particularly enjoying card games and dominoes. Although the beginning of his journey presented difficulties, he demonstrated resilience and determination, which contributed to steady progress.

Over time, Mr. S made a remarkable turnaround and began to excel in all areas of therapy. He showed significant improvement in his ability to transfer independently from bed to chair. Currently, he is able to get out of bed and into his chair without assistance. He has progressed to standing independently with supervision to ensure safety and prevent falls.

Additionally, Mr. S can now transfer from his chair to the toilet independently, with staff nearby for supervision. His increased strength, balance, and confidence have greatly improved his independence in daily activities. Notably, he demonstrated significant upper body strength gains, progressing from being unable to raise his shoulder past 90 degrees of flexion to achieving 150 degrees of flexion while utilizing 6-pound weights. He continues to demonstrate motivation and commitment to his recovery, consistently meeting and exceeding therapy goals.

Conclusion: Mr. S’s progress has been exceptional. Due to his significant improvement and success in therapy, our prosthetics specialist, Mr. Hanger, met with him to begin discussions regarding fitting his left prosthetic leg. At this time, our team is actively preparing him for this next phase of his rehabilitation. Once he is successfully fitted and trained with his prosthetic, the plan is for Mr. S to discharge home, where he will reside with his sister. His journey reflects the impact of coordinated, patient-centered care and his own determination to regain independence.
