How did this resident hear about The Emerald Peek: Phelps Hospital Northwell Health
Details of Experience:
Lawrence was referred to our community through Phelps Hospital, Northwell Health, for comprehensive sub-acute rehabilitation services following a cerebrovascular accident (stroke) that resulted in right-sided hemiparesis and aphasia. The stroke caused a significant decline in his mobility, functional independence, and communication abilities, requiring an interdisciplinary approach to help him regain strength and confidence.
Lawrence was admitted to The Emerald Peek Rehabilitation and Nursing Center with diagnoses of right hemiparesis, aphasia, generalized muscle weakness, impaired mobility, and difficulty performing daily activities. Upon admission, he was non-ambulatory, dependent on a wheelchair for mobility, and required dependent assistance from two staff members for sit-to-stand transfers. His rehabilitation plan included both skilled Physical and Occupational Therapy designed to restore his independence and improve his overall quality of life.
From the moment Lawrence joined our community, he demonstrated remarkable determination despite the challenges he faced. Although aphasia affected his speech, it never prevented him from communicating with those around him. Through gestures, facial expressions, and consistent effort, Lawrence built meaningful connections with staff and fellow residents. His positive attitude and willingness to participate made him a truly valued member of our community.
Lawrence participated in skilled Physical Therapy five days per week, focusing on progressive strengthening, bed mobility, transfer training, balance activities, and functional mobility. As his endurance and strength improved, his therapy team safely introduced gait training with a walker. Occupational Therapy complemented his progress by focusing on activities of daily living (ADLs), helping him regain the skills needed for greater independence in everyday tasks.
The beginning of Lawrence’s rehabilitation journey presented many obstacles. His right-sided weakness made even the simplest movements difficult, and standing required extensive assistance. However, through perseverance and the unwavering support of his dedicated therapy team, Lawrence continued to make meaningful progress with each passing week. His therapists frequently recognized his commitment, resilience, and eagerness to improve, noting that he approached every therapy session with determination and a positive mindset.
One of Lawrence’s greatest accomplishments was progressing from complete dependence on a wheelchair to walking independently throughout our halls with a walker. This milestone reflected not only his improved strength and balance but also the confidence he had developed through months of dedicated rehabilitation. Watching him achieve this level of independence was an incredibly rewarding experience for everyone involved in his care.
Lawrence’s physical abilities improved, and so did his engagement within the community. He became familiar and friendly with his face throughout the building, enjoying conversations and interactions with fellow residents despite the limitations of his aphasia. He especially enjoyed spending time outdoors, often sitting outside to appreciate the beautiful weather, and regularly participated in community activities and events. His warm personality and positive outlook made him a pleasure to have in our community, and he formed lasting relationships with both residents and staff.
Throughout his stay, Lawrence remained appreciative of the therapists, nurses, CNAs, Concierge, and every member of the care team who supported his recovery. His determination, resilience, and steady progress served as an inspiration to everyone who had the privilege of working with him.
Lawrence continues to be part of the community here at The Emerald Peek, walking confidently with his walker. His rehabilitation journey stands as a testament to the power of determination, skilled therapy, and compassionate care working together to achieve meaningful outcomes.
