When Rachel I. was admitted to Glengariff Rehabilitation and Healthcare Center from New York Presbyterian Hospital–Columbia on January 7, 2026, she required extensive assistance due to the severity of her injury and resulting functional limitations. Upon admission, Rachel was dependent on a Hoyer/mechanical lift for transfers and was unable to safely perform mobility tasks independently. In occupational therapy, she required partial to maximum assistance with daily activities, while in physical therapy, she was fully dependent for mobility and transfers.

Prior to her injury, Rachel had been living independently in her apartment. Following her femur fracture, regaining her strength, mobility, and independence became the primary focus of her rehabilitation journey. The interdisciplinary team at Glengariff, including physical and occupational therapists, nursing staff, physicians, and support services, developed a personalized care plan designed to help Rachel return to her highest level of function.

Throughout her stay, Rachel remained motivated and committed to her recovery goals. Working closely with the rehabilitation team, she participated in daily therapy sessions focused on improving strength, balance, endurance, transfer skills, and ambulation. Through consistent effort and skilled therapeutic intervention, Rachel made remarkable progress.

By the time of her discharge on April 16, 2026, Rachel was able to ambulate 150 feet, safely navigate stairs, and independently transfer in and out of chairs. Her progress represented a significant transformation from her condition upon admission. Most importantly, Rachel regained the functional independence necessary to safely return home and resume living alone in her apartment.
