How did this patient hear about Creekside Center? Case manager at Skyline Medical Center
Details of Experience:
Mr. G was admitted to Creekside Center for Rehabilitation and Healing following a hospitalization at TriStar Skyline Medical Center for osteomyelitis of the left ankle and foot, a serious infection of the bone, and cellulitis of the lower limb, a bacterial infection of the skin and underlying tissues that caused redness, swelling, warmth, and pain. Upon admission, he was non—weight bearing on his left foot, making mobility and daily activities challenging while he focused on healing.
Mr. G shared that his only goal was to walk again. During his stay, he worked closely with our interdisciplinary team to regain his strength and independence. Physical Therapist Assistant Joe and Occupational Therapist Katie developed an individualized rehabilitation plan focused on improving mobility, restoring functional independence, and preparing him for a safe return home. Through consistent therapy and his own determination, Mr. G made meaningful progress throughout his recovery. By the time of discharge, Mr. G had progressed from non-weight-bearing to safely ambulating with a walker, a significant milestone that allowed him to return home with increased independence and confidence alongside his wife.
Reflecting on his rehabilitation journey, Mr. G shared, “This was definitely a new experience, and I was curious as to what it would hold. Joe and Katie were nothing short of terrific. I’m truly grateful for their support and all of the work that they did with me.”
Mr. G’s journey is a testament to the power of personalized rehabilitation and the dedication of a rehabilitation team committed to helping each patient reach their goals. Through excellent therapy and his unwavering determination during his 34-day stay, Mr. G made remarkable progress and was able to safely return home on June 21, 2026, to continue the next chapter of his recovery. Creekside Center for Rehabilitation and Healing wishes Mr. G continued strength, good health, and many happy moments at home with his wife. The team is grateful to have been a part of his recovery journey and wishes him all the best in the future.
