Details of Experience:

Ercel was admitted to Sumner Regional Medical Center on April 29th, 2026, due to increased weakness, ambulatory dysfunction, and diarrhea for three days. She was admitted and treated for renal failure/ dehydration due to diarrhea, as well as multiple other acute and chronic medical problems, which were addressed by SRMC staff. Once stabilized, Ercel was referred to the Gallatin Center for Rehabilitation and Healing for skilled services, including physical and occupational therapy.

Ercel S. was admitted to the Gallatin Center on April 30th, 2026. Upon admission, Ercel was welcomed by the concierge and introduced to her therapy team, nursing team, social services, activities director, and dietary director, and a plan of care was created. Ercel demonstrated decreased strength, impaired balance, difficulty walking, and increased need for assistance with activities of daily living.

Skilled therapy services were started to restore function, improve safety, and maximize independence. Ercel participated in therapy exercises, gait training with a rolling walker, transfer training, balance activities, endurance training, wheelchair mobility training, and ADL retraining. At the start of therapy, Ercel required partial to moderate assistance with bed mobility, dressing, bathing, and personal hygiene, and was unable to ambulate functional distances. Through much participation, Ercel made much progress in strength, balance, endurance, and functional mobility. She progressed to independence with eating and oral hygiene, and to setup assistance for bed mobility, bathing, dressing, and wheelchair mobility, and is able to ambulate 100-150 feet with a rolling walker and contact guard assistance. These improvements have enabled her to participate in daily activities while reducing caregiver assistance.

Ercel has achieved her highest level of function and has met her skilled therapy goals. Ercel stated she would “recommend the therapy department here for anyone to go to.” When asked how she felt the team had helped her, she stated, “They are wonderful people, and have helped me more than they know.” Ercel will be discharged from therapy services while remaining a long-term care resident at Gallatin Center for Rehabilitation and Healing. Her functional gains have improved her safety, mobility, and overall quality of life, and facility staff will continue to support her daily care needs and help maintain her current level of function.
