Patient was admitted to St. James Rehabilitation Center after a lengthy stay at SBUH due to Acute Kidney Injury with hypotension and Osteomyletis . She was placed on a rigorous course of IV antibiotics in which she required a central line to be placed and monitored. Upon her admission, she had weakness, difficulty transferring, difficulty ambulating, and required assistance when performing self care. She attended physical therapy six days a week for therapeutic exercise, gait training, balance, therapeutic activities. She also attended occupational therapy six times a week for , self care, transfers and ADLs. During her stay she was followed by the in house pulmonologist due to her past medical history of COPD. Medications were adjusted accordingly. In addition, she was monitored by her primary care physician weekly. Her central line and wound vac was maintained and cared for by the nurses on the floor. Her subclavian wound showed signs of healing; she was educated on wound care in order to maintain healing when returning to the community. She completed her course of antibiotics and her PICC line was removed. Prior to returning to the community she received education on medication administration, potential side effects, and how to obtain her blood sugar. Upon discharge great improvements had been made in her ability to perform self care, ambulate, transfer, and safety.
