How did you hear about Chatham Hills? Patient was here previously for rehabilitation
Patient was admitted to Chatham Hills Subacute Care Center after his stay in Morristown Memorial Hospital due to Syncope and Collapse; Difficulty in walking; Muscle Weakness. At the time of admission, patient was very weak and needed improvement with walking and building endurance. It was important for the patient to achieve several goals before being discharged. These included lower extremity strengthening and balance. This was achieved with seated leg exercises, standing exercises and sequencing for stair negotiation. Rehab worked with him twice a day and successfully reached the goal of discharging the patient with the prior level of function. This patient was so pleased, yet again, with his care he was happy to give us a glowing testimonial.
