How did this patient hear about Ramapo Manor? Case Management, Wayne Hospital

Details of Experience:

Erick, a young man, was admitted to Ramapo Manor Center for Rehabilitation on November 29, 2016 after a two month inpatient hospitalization in New Jersey. Erick was diagnosed with muscular dystrophy and required support for a post – tracheotomy from sever pneumonia. Erick’s intentions after discharging from the hospital was to return home; however, his family sought out education for trash care and oxygen programming prior to a safe discharge being made. The respiratory therapist at Ramapo Manor were very excited and humbled for the opportunity to support Erick and his family. During Erick’s rehabilitation journey at Ramapo Manor, Erick’s family were introduced to clinical training programs to provide comprehensive education for tracheotomy care.

Education included information on daily changing of all disposable equipment and use of the oxygen concentrator, suctioning procedures and literature on ancillary materials required for quality of care once returning home. Erick shared, “I am so happy with the care I got at Ramapo Rehab; the best part is I get to go home to be with my family”. Erick was successfully discharged home to be with his family on January 5, 2017. The Ramapo Manor interdisciplinary team were instrumental in Erick’s recovery and we would like to recognize all for their devotion. Erick currently resides with his family who also cares for his brother whom has also been diagnosed with muscular dystrophy.
