Details of Experience:

Jean was presented to Morristown Medical Center on September 13, 2017 with complaints of expressive aphasia and confusion noted by her family. She went through various tests that confirmed that she had a stroke. She was discharged to an acute care facility where she stayed for a couple days until she had to be readmitted to the hospital again due to shortness of breath, worsening speech and generalized weakness. She was discharged back to an acute care facility where she stayed for about two weeks. She was then informed that she would need extensive subacute care to be able to go back to her prior level of function.

Upon arrival to Chatham Hills Subacute Care Center, Jean was greeted by professionals who anticipated her arrival. Within the first twenty-four hours of her stay, she was evaluated by her therapists to determine the level of care she was going to need. Jean came across multiple complications throughout her stay at Chatham Hills. Just weeks after her admission, she was sent to the hospital due to increasingly slurred speech and having trouble bringing the fork to her mouth while she ate. Upon returning back to Chatham Hills, Jean was presented with weakness and a significant decline in mobility and self-care.

Jean made consistent progress and substantial functional gains in response to skilled interventions. When she was initially admitted to Chatham Hills, she needed assistance with all ADL’s-(activities of daily living). Jean also was diagnosed with Dysphagia. Her swallowing was impaired with right sided facial weakness. Upon admission, she was on a puree diet with thickened liquids. While receiving speech therapy three times a week, she started showing substantial progress. With exercise, her facial weakness improved, her vocal hoarseness improved and she advanced two diet levels!

On December 8, 2017, Jean was discharged home with an eager excitement to be back with her very supportive family. Upon discharge, her ambulation increased from 150 feet to over 300 feet and she was able to be independent achieving her goal. She only needed minimal supervision with a regular diet, thin liquids and being able to feed herself!
