How did this patient hear about The Chateau at Brooklyn Rehabilitation & Nursing Center? Social Worker at SUNY.
Anne was admitted to The Chateau at Brooklyn Rehabilitation and Nursing Center after an unfortunate circumstance resulting in her breaking her femur while residing at home at her home. She was living alone prior to her fracture and wanted to be able to continue living independently. Surgery was performed to repair her femur during her stay in the hospital. Post-surgery rehabilitation would be needed in order to obtain her independent lifestyle again.
Anne wanted to pick a rehabilitation community that was close to her house and that would also have the capabilities of helping her reach her goals. After speaking with the social workers at the hospital, they recommended that she transfer to a location that she will be very happy rehabilitating at, The Chateau at Brooklyn Rehabilitation and Nursing Center.
Upon arrival at The Chateau, Anne was very nervous about how her stay would go. She was never in a rehabilitation community before and wasn’t sure what to expect. Anne soon met the staff members who explained to her the process of how things would go and explained that everything would be fine.
Anne became calmer and was looking forward to starting her therapy. Before beginning therapy, Anne was unable to walk on her own and needed assistance to transfer from standing to sitting. A concern that Anne had was that she would need to be able to walk the steps that she had in her house. The staff insisted that Anne will be prepared on the day of her discharge.
As soon as Anne began working in the gym, she became a new person. She was excited about seeing such great results and started looking forward to her discharge. After working consistently with her therapists for a few weeks, Anne was able to walk on her own without any assistance or devices and was able to climb the steps independently as well. When she reached all her milestones and goals, the staff were happy to help set up her discharge.
After three weeks of hard work, Anne was able to go home and continue living life as she once did before her fall. The staff at The Chateau were happy to see her walk out on her own and one staff member mistook her for a visitor and asked her to sign out from the guest book. That made Anne smile, “I came as a patient and left as a visitor and friend.”
From all of us at The Chateau, we wish Anne much luck and we are excited for her to back to living comfortably in her home!
