Details of Experience:
On Christmas Day, after being ill for three days, Ms. Annie Bunker was transported by ambulance to Skyline Medical Center. She presented with altered mental status and critically low oxygen levels due to Pneumonia. Initially, family members were warned that she may not revive. After the administration of steroids, antibiotics and breathing treatments, she began to improve. Prior to the onset of symptoms, Ms. Bunker was able to care for herself and function independently alone in her home. This was no longer possible without continued therapy, assistance and nursing care at a skilled rehab center. Thus, Ms. Bunker was referred to Creekside Center for Rehabilitation and Healing.

Upon admission, Ms. Bunker was kindly greeted by staff who worked as a team to ensure she was comfortable, and her needs and preferences were met. Her initial therapy evaluations indicated one-person assistance with the use of a walker was required for ambulation and activities of daily living. She experienced generalized weakness and shortness of breath that only allowed her to walk short distances. Ms. Bunker was optimistic and determined to work towards her goal to increase strength and endurance in order to regain her independence.

About a week after receiving therapy, her progress was considerable. She was more energetic and able to walk 150 feet. Performance of daily tasks was accomplished with supervision. When asked about her experience at Creekside Center, she said, “everyone has been knocking it out of the ball field.” She was most surprised about the outstanding care and how staff members were incredibly friendly and encouraging. In her free time, she attended various activities. The music entertainment and playing Bingo were her favorites. The coffee and snacks offered by staff were enjoyed, and she looked forward to discovering what foods would be served at mealtime.

As Ms. Annie approached her discharge, her cooperation with therapy was paying off. She began walking 300 feet without exhaustion or the need for standby assistance. She fulfilled her daily tasks with modified independence. Overcoming twelve stairs was another one of her many accomplishments. Goals were met and exceeded. Ms. Annie became excited about going home to spend more time with her family who lived nearby. Cooking, going to church and reading are some of the things she will be doing. She announced, “I hate to leave everyone, but I will be coming back to visit.” The Creekside Center team enjoyed our time with her and wish her continued success.
