Details of Experience:
Ms. Johnson was admitted to The Phoenix Rehabilitation and Nurse Center on January 17, 2020 from The Brooklyn Hospital where she had been admitted due to a motor vehicle accident in which she injured her neck and back. During her first 24 hours in the community, Ms. Johnson was welcomed by the community staff. She required extensive assistance with personal hygiene, dressing, grooming and bathing, and needed a two person extensive assistance for transfers.

Ms. Johnson had challenges resting well at night due to pain from her injuries. The team quickly worked together to develop a pain management program which enabled Ms. Johnson to rest more comfortably. Physical and Occupational Therapy developed individualized treatment plans to enable Ms. Johnson to reach her maximum potential. She was able to complete her rehabilitation, reaching her goals, with minimal pain and discomfort.

Both the Therapeutic Recreation team and Concierge teams were able to provide Ms. Johnson with independent and group activities to support her psycho-social well being. She participated in religious services and Bingo weekly. She enjoyed independent activities as well, such as crossword puzzle books and daily visits from her Personal Concierge.

On February 20, 2020, Ms. Johnson was discharged to the community. Her Social Worker reached out to Ms. Johnson post-discharge where Ms. Johnson shared she was doing well. The team at The Phoenix wishes Ms. Johnson the best and a long, healthy life.
