Details of Experience:
Patrick B. was admitted to The Emerald Peek Rehabilitation and Nursing Center at the start of September of 2020. Mr. B. had encountered a fall in Hedgewood Assisted Living Facility where he resided, due to which he was taken to St. Luke’s Cornwall Hospital. Upon evaluation in the hospital, it was discovered that Mr. B. had sustained an injury in the back of his head. When he first came to The Emerald Peek Rehabilitation and Nursing Center, on September 3rd, he was greeted by the Concierge, who upon talking to Mr. B., noticed that he was very confused and had trouble remembering things and concentrating on the information given to him. Patrick was suffering from a condition where due to the chemical imbalance in the body, neurological impairments start developing, leading to confusion, difficulty remembering things, changes in personal behavior, and in some people, it even causes balance issues. Additionally, he was also having trouble breathing, due to which he required to be given oxygen during the night time. However, as he started becoming accustomed to his new surroundings, he was able to bring his needs to the attention of the staff here at The Emerald Peek, without any major limitations.

Patrick’s initial rehabilitation evaluation at The Emerald Peek was completed by Khristine from Physical Therapy and Julio from Occupational Therapy. Upon evaluation, it was clear that Patrick required extensive assistance for most of his activities of daily living. Although he was able to independently feed himself, he did not feel confident with performing basic tasks like ambulating, going to the toilet by himself and showering independently. All he wanted was to regain his strength to be able to perform all these tasks. In accordance to what he wanted, the rehab team here at The Emerald Peek, formulated a treatment plan where he would receive restorative Occupational therapy exercises, as well as skilled Physical therapy exercises including re-training for his Activities of Daily Living, therapeutic exercises, neurological re-education, and gait training to improve his balance. His Short term goals included increasing his strength and endurance through a series of retraining exercises in the community’s well-equipped rehab gym, while his long term goal was to improve his functional mobility in order to be able to independently perform his activities of daily living.

Although being initially nervous about his stay here, he was always willing to try the recreational activities offered to him. Slowly he started enjoying his time here due to the various options for leisure activities provided to the residents at The Emerald Peek. Being a frequent admirer of good music, Mr. B. was often seen with his headphones on, in his room, listening to music while enjoying a snack from the snack cart, or an ice-cream cup from the ice-cream cart. Occasionally, he would also participate in the happy hour program provided by the activities team. His favorite program of all time, he shared with the Concierge, was the hand massages provided using essential oils, as well as the lavender-scented hot towel massages.

Slowly and steadily, his strength and endurance started to improve. He received constant encouragement and re-education from the rehab staff regarding safety awareness. An important part of his treatment process was the ambulatory re-training while maintaining his body’s balance. However, he was motivated to reach his goals and therefore, he closely followed every instruction given to him by his therapists. This motivation led to him being able to ambulate 300-350 feet with modified independence with the help of a rolling walker, which was huge progress from his initial ability to ambulate a mere 10 feet with modified assistance, with the help of a rolling walker, when he first came to The Emerald Peek. When he finally was ready to go back to Hedgewood after a month, on October 1st, 2020, he was so happy with his stay here at Emerald Peek, that he rated his overall satisfactory level, a maximum of 10 out of 10 in his discharge survey.
