Details of Experience:
Mr. Jones was admitted to Lebanon Center on February 5th, 2021, with the diagnosis of metabolic encephalopathy and severe swelling of both feet. Mr. Jones had generalized weakness and a history of falls. Upon arriving at Lebanon Center, he was evaluated by our therapy department, during which they discovered he was fragile and needed maximum assistance for transfers and was unable to walk.

After a week of therapy, he was transferred with the assistance of one person, when upon admission, he required two. He also was now able to stay up most of the day. The second week, he was standing 2-3 minutes with the assistance of one person. In week three, he walked short distances with moderate assistance. Then moving into the next two weeks, he was able to walk 50-100 feet with much-improved gait mechanics and just stand by assist, also standing on his own during the day to practice using the grab bar.

Before leaving the community, Mr. Jones could walk 100-200 feet with a walker and just supervision. Mr. Jones was now 90 percent independent with all daily tasks and returned home to his house with assistance from family. Before leaving the community, Mr. Jones told a staff member that he was so happy with his stay that he ”wanted to come volunteer when covid restrictions are lifted.”
