How did the patient hear about Sans Souci Rehabilitation and Nursing Center? Hospital Referral

Details of Experience:
Ms. Newman arrived at beautiful Sans Souci on May 4, 2021. Upon arrival, the patient was very eager and expressed her wish to go home. Our dedicated concierge was one of the first to pay a visit to her room and sat and listened to the concerns of our new patient. Our concierge was caring and compassionate and comforted Ms. Newman making her comfortable like she was at home.

Our dedicated rehab team saw the patient and together created an achievable goal. Ms. Newman had also shared that the entranceway to her apartment had four steps, and to get back to normal life after rehab, she needed to climb at least four steps to come and go freely and easily.

Upon admission, the patient was unable to ambulate on a level surface more than 5 feet, to perform functional transfers on her own and balance on her own without pain, to perform bed mobility, meaning sitting up or getting in or out of bed by herself safely due to weakness and unsteadiness on feet.  The agreed-upon goals included: to be able to climb a minimum of four steps, to perform toileting tasks using a commode for safety, to perform safely and efficiently dressing with the use of a dressing stick and a sock aid stick, to perform functional transfers with minimum assistance, and safely ambulate on a level surface 175 feet.

The following morning, Ms. Newman was up and ready and patiently awaiting the arrival of our dedicated rehab team with no time to waste. Ms. Newman showed real passion and eagerness. On day three,  Ms. Newman was getting a bit impatient and once again expressed her willingness to go home. Our team sat with the patient and explained to her that it might seem to her that she is ready to be discharged, although in reality, she just needs a little more rehab, and she would be on her way. With the bond and trust that they developed, Ms. Newman kept going and progressing towards her discharge date.

On May 14th, Ms. Newman was discharged home to her own house and was delighted to climb those steps in front of her house by herself. Upon discharge, Ms. Newman was able to climb four steps easily, to safely perform toileting tasks using toilet seat commode for better assistance, to perform safely and efficiently dressing with the use of a dressing stick and a sock aid stick, to perform functional transfers with minimum assistance and safely ambulate on level surface 200 feet walker, 25 additional feet from her initial goal!

Congratulations, Ms. Newman!
