How did the patient hear about The Enclave at Rye: Recommended by her surgeon

Reason for Stay: Rehabilitation following surgery

Details of Experience:

Concetta was admitted to The Enclave at Rye Rehabilitation and Nursing Center on October 15, 2022, after being referred by White Plains Hospital. Upon arriving at our community, Concetta and her son were welcomed by the concierge and introduced to the interdisciplinary care team, including her nurse and doctor.

On October 11, 2022, Concetta fell down the stairs at home and experienced left knee pain. She was taken to Greenwich Hospital for x-rays, which revealed a fracture in her left patella. The orthopedic surgeon recommended a partial patellectomy and instructed Concetta to be non-weight-bearing on her left lower extremity, wearing a knee immobilizer at all times.

After the surgery, Concetta experienced new-onset deficits in muscle strength, endurance/activity tolerance, and standing balance/standing tolerance. She was noted to have decreased endurance and activity tolerance due to pain in her left knee, requiring frequent rest breaks during activities. The therapy team conducted an evaluation and developed a plan to address Concetta’s specific needs. They determined that Concetta would benefit from skilled physical therapy (PT) to improve bed mobility, transfers, ambulation, stair training, lower extremity strength, sitting and standing balance, activities of daily living performance, and patient education in safety awareness to decrease her fall risk. Without skilled PT, Concetta was at risk for falls, contractures and would be unable to return to her prior way of life.

Dr. Abbey and the nursing team were dedicated to ensuring Concetta received the best possible care during her stay. They kept a close eye on her and made sure all of her needs were met, including administering pain medication and addressing any medical problems that arose. This close attention allowed Concetta to maximize her time in the therapy gym, where she worked with the physical therapy team to improve her mobility, strength, and function.

In addition to her physical therapy sessions, Concetta also enjoyed participating in the recreational activities offered at The Enclave. She could often be found joining the recreation team for daily activities, such as entertainment and crafts. Concetta was known for her enthusiasm and eagerness to participate, and she quickly became a beloved member of the community at the nursing home. She made many friends and was always seen enjoying herself during these activities. The recreational programming at the nursing home played an important role in her overall recovery and well-being.

Throughout her stay at The Enclave, Concetta made steady progress in her recovery. With the help of the skilled staff and therapy team, she was able to regain her strength and independence. She was also able to utilize our very own HydroWorx equipment to maximize her strength. At this time, Concetta no longer requires assistance with her activities of daily living and is able to return to her prior way of life. Concetta worked hard and was dedicated to her recovery, and the results were clear. She enjoyed her time at the Enclave and is grateful for the support and care provided by the staff. The Enclave at Rye staff were happy to have been able to help Concetta on her journey to recovery and wish her well as she begins to move forward.
