Details of experience:

Thomas arrived at the Quality Center for Rehabilitation and Healing on January 29, 2025. He was living independently prior to his accident. Mr. R was in his kitchen when he felt weakness in his legs, his right leg ultimately giving out, and he subsequently fell and fractured his right ankle on January 23, 2025. He was then taken by EMS to Rutherford in Murfreesboro, Tennessee, to be examined and treated. Mr. R had surgery while there to repair the damage caused to his right ankle the following day on January 24, 2025. 

Upon his arrival in our community, Mr. R was greeted by several members of our professional team. Within his first 48 hours here, he met with our clinicians, therapists, physicians, administrative staff, and many other team members, who welcomed him to our community, assuring him they were all here to help him in his recovery. He was also greeted by our concierge, Cecelia Hall, who gifted him with a branded Care Rite welcome bag, explained her role as liaison, answered many questions, and spoke with him at length about his life prior to his accident and what his goals were here at Quality Center for Rehabilitation and Healing. 

After his admission to our community, Mr. R was evaluated by Therapy on January 30, 2025. Physical therapy noted that he had difficulty walking and that he was able to ambulate 50 feet with supervision. He then met with Occupational Therapy and it was noted that Mr. R was maximal assistance for toilet transfers, his ability to efficiently maintain hygiene was complete dependence, maximal assistance for lower body ADLs, his ability to stand supported for safety with mobility tasks was less than 30 seconds, and his functional transfers were not attempted at that time due to health conditions and safety concerns. 

During his time here in our community, working with our rehabilitation team, Mr. R was able to make progress in his recovery and meet the goals that were set for him. Physical Therapy noted that by March 19, 2025, Mr. R was now able to ambulate 200 feet with set-up assistance. Occupational Therapy noted that by that time, he was now capable of independent toilet transfers, maintained personal and perineal hygiene independently, his lower body ADLs were now set up assistance, his ability to stand supported increased from less than 30 seconds to 5 minutes, and his functional transfers were deemed supervision assistance. 

Mr. R was a part of our community for 54 days and was discharged on March 25, 2025. Before he left, he shared that his goal in coming here was simply to get better, return home, and continue to live independently. While he did return home with some assistance from Adoration Home Health.  He stated that he was leaving here satisfied and that he feels working with our rehabilitation team accomplished what it was supposed to for him.
